Privacy Policy

Pasadena Clinical Group ("we," "our," or "us") respects your privacy. This Privacy Policy explains how we collect, use, disclose, and safeguard information in connection with our website (LAAnxietyGroups.com) and the administrative business operations that support our practice. It is intended to comply with the California Consumer Privacy Act of 2018 (CCPA) as amended by the California Privacy Rights Act of 2020 (CPRA), as well as with applicable federal law.

This Privacy Policy applies to information collected through our website and general business operations. It does not govern Protected Health Information ("PHI") collected during treatment, which is described in our Notice of Privacy Practices (HIPAA) and protected by HIPAA, the California Confidentiality of Medical Information Act (CMIA, Civil Code §56 et seq.), and California Welfare & Institutions Code §5328 where applicable.

1. Categories of personal information we collect

In the past twelve (12) months, we may collect the following categories of personal information, as defined in Cal. Civ. Code §1798.140:

  • Identifiers: name, email, phone number, postal address, IP address, online identifiers.
  • Customer records (Cal. Civ. Code §1798.80(e)): contact information voluntarily submitted via our forms.
  • Internet/network activity: browsing behavior on this site, pages viewed, time on page, referring URL, device characteristics.
  • Geolocation: general (city/region) inferred from IP address only — we do not collect precise GPS location.
  • Sensitive personal information (CPRA category): information you choose to share that may concern your mental or physical health (e.g., a free-text message describing what brought you to our site). We treat such information with elevated care and use it solely to respond to your inquiry.
  • Inferences: we do not generate behavioral profiles for advertising purposes.

2. Sources of personal information

  • Directly from you (forms, calls, emails).
  • Automatically through your interaction with the website (cookies, analytics — only with consent).
  • From service providers acting on our behalf (e.g., Cloudflare for security and form delivery, our scheduling system).

3. Business and commercial purposes

  • To respond to your inquiry, schedule appointments, and provide care coordination.
  • To verify insurance, collect payments, and conduct standard healthcare operations.
  • To detect and prevent security incidents, fraud, and unauthorized activity.
  • To comply with legal obligations.
  • To improve our website, with your consent for analytics.

4. Categories of third parties with whom we share information

  • Service providers bound by contracts that restrict their use to the purposes for which we share data — including our website host, our forms processor (Cloudflare Worker forwarding to internal automation), our practice-management/EHR vendor, and our payment processor.
  • Government authorities, only when required by law (subpoena, court order, mandated reporting).
  • Insurers and billing partners, as authorized by you for the purpose of claims and payment.

5. Sale or sharing of personal information ("Do Not Sell or Share")

We do not sell personal information for monetary value. We do not "share" personal information for cross-context behavioral advertising as that term is defined under CPRA. If our practices change, we will update this Policy and provide a clear opt-out mechanism. You can also access your cookie preferences and "Do Not Sell or Share" controls here.

6. Sensitive personal information

You have the right under CPRA to limit our use of sensitive personal information to what is necessary to provide the services you have requested. We honor this right by default — we use any sensitive information you submit only to respond to your inquiry and provide care, and we do not use it for inference or profiling.

7. Your privacy rights under CCPA / CPRA

If you are a California resident, you have the right to:

  • Right to know what personal information we have collected about you, the sources, the purposes, and the categories of third parties to whom it has been disclosed (Cal. Civ. Code §1798.110).
  • Right to delete personal information we have collected (subject to exceptions such as healthcare records governed by HIPAA/CMIA) (§1798.105).
  • Right to correct inaccurate personal information (§1798.106).
  • Right to opt-out of sale or sharing (§1798.120).
  • Right to limit use of sensitive personal information (§1798.121).
  • Right to non-discrimination for exercising any of the above (§1798.125). We will not deny services, charge different prices, or provide a different level of quality because you exercised a privacy right.

8. How to exercise your rights

To submit a request, email office@pasadenaclinicalgroup.com with the subject line "CCPA/CPRA Request," or call (626) 354-6440. We will verify your identity (typically by matching information you provide against our records) before fulfilling the request and respond within 45 days. If we need additional time, we will notify you within 45 days and provide our response within 90 days total. You may designate an authorized agent to make a request on your behalf by providing the agent with written, signed permission and verifying your identity directly with us.

9. Cookies and tracking technologies

We use cookies and similar technologies as described in our Cookie Policy. Non-essential cookies (analytics, functionality, marketing) are off by default and load only after you provide consent. You can change your cookie preferences at any time using the Cookie Preferences link in the footer.

10. Children's privacy

Our website is not directed to children under 13, and we do not knowingly collect personal information from children under 13 in violation of COPPA. California law applies a heightened standard to information of consumers under 16 — we do not knowingly sell or share information from minors. If you believe we have inadvertently collected information from a child under 13, please contact us so we can delete it.

11. Data retention

We retain personal information collected through this website only as long as needed for the purposes described above and to comply with legal obligations. Retention of healthcare records is governed by separate state and federal requirements described in our Notice of Privacy Practices.

12. Security

We use reasonable administrative, technical, and physical safeguards to protect personal information. No method of transmission over the internet is fully secure. Please do not include sensitive personal health information in our website forms — call us at (626) 354-6440 for sensitive matters.

13. Changes to this Privacy Policy

We may update this Policy from time to time. The "last updated" date at the top of this page reflects the most recent change. Material changes will be highlighted in a notice on the site for at least thirty (30) days.
